Judge Advocate Commissions
Note: The guidance provided in the ANG Checklist for Web Applications is also helpful in identifying forms that need to be submitted with an application for a ANG position.

AF Fm 1288, Application for Ready Reserve Assignment (AF/USAFR) or a Request for Conditional Release from another component - A letter requesting and authorizing conditional release may suffice in lieu of AF Fm 1288. Conditional release is only required for officers transferring to the ANG from another component.
AF Fm 2030, USAF Drug and Alcohol Abuse Certificate - Except for the first question, all “yes” blocks initialed, must be explained by applicant. All blocks must be initialed.
Items below not currently available in electronic format (must be provided by applicant ):
DD Fm 785, Record of Disenrollment from Officer Candidate Training - Required for applicants eliminated from a commissioning program. If applicant was disenrolled from AFROTC after the second year, a DD Fm 785 must be provided
DD Fm 214, Certificate of Release/Discharge from Active Duty - Required for individuals with prior active duty.
AF Fm 357, Dependent Care Certification - All applicants with dependents. Approved by Commander
Certificate of Citizenship Statement - Naturalized citizens only. Duplication of citizenship card is not authorized. Annotate (or type) naturalization certificate number on AF Form 24.
Statement of Agreement and Understanding - Must use form at Atch 2.
Certificate of Admission to Bar, current legal standing
Interview by Active Duty Judge Advocate
Consolidated Certified Transcript of College Credits or Professional Training
Prior Service Records - Includes appointment, promotion and discharge orders/oaths-If applicable
